All persons making an application to join the practice must do so by requesting an application form from our reception staff. Application forms can be obtained by clicking on the links below. However, if you complete and return the registration forms and you do not reside within the practice boundary you will be informed that we are unable to accept your registration at the practice.
Please check with our reception staff to ensure you live within the boundary area.
Upon full completion, the form should be returned to reception with proof of address and ID.
A new patient health check appointment will be made if required with the practice nurse.
From the 1st April 2004, all patients will be registered with the practice and not with a specific doctor, patients may express a preference to see a particular doctor when making appointments.
In the event that the doctor of your choice is unavailable, then we will offer you an appointment with another GP.
If a patient insists on seeing a specific doctor then this may result in waiting longer for an available appointment.
Both patient registration forms must be completed ( patient GSM1 form and new patient information form)
