New Patient Registration

All persons making an application to join the practice can do so by requesting an application form from our reception staff or you can download the forms by clicking on the links below. However, if you do not reside within the practice boundary you will be informed that we are unable to accept your registration at the practice. Please check with our reception staff to ensure that you reside within the boundary area or check the map below.

From 24th January 2022, The Marshside Surgery (along with other local surgeries) will not be registering new patients who are already registered with a GP surgery locally, & who still live within that GP surgery's registered practice area. Please find out the reasons why this decision was necessary from our news page. Please support your GP practices at this time; if you are already registered with a GP in this area we would ask you to consider whether your decision to move practices could be deferred.

Upon full completion of all the new registration forms, the form should be returned to reception with proof of address and ID.

A new patient health check appointment will be offered upon registration with the practice nurse or HCA.

From the 1st of April 2004, all patients will be registered with the practice and not with a specific doctor. Patients may express a preference to see a particular doctor when making appointments, but this may result in waiting longer. Patients will always be offered an appointment with an appropriate healthcare professional.

Both patient registration forms must be completed (patient GSM1 form and new patient information form)
